Abstract

We present the case of a 58-year-old male patient from Santiago del Estero, a rural worker, who consulted for an inflammatory tumor involving skin and deep tissues of the right foot, which has an estimated evolution of two years. Microbiological cultures showed dry and cerebriform colonies, which were identified as Streptomyces sp. by MALDI TOF mass spectrometry, BD Bruker ® and whole genome sequencing. The condition was diagnosed as actinomycetoma. Imaging studies showed bone involvement, so treatment with trimethoprim-sulfamethoxazole and minocycline was initiated. However, due to the development of pulmonary tuberculosis, treatment was adjusted to rifampin, ethambutol, pyrazinamide, isoniazid, and trimethoprim-sulfamethoxazole, with favorable clinical outcome.
